Wedge-Shaped Defect (Cervical Abfraction)

Overview

A wedge-shaped defect is a wedge-shaped loss of hard tissue at the labial or buccal cervical region of the tooth, often related to improper horizontal brushing, acid erosion, and concentrated occlusal stress. It presents as a defect at the tooth neck that can cause sensitivity, and when too deep may expose the pulp or cause tooth fracture.

Symptoms

• Concave defect at the tooth neck
• Sensitivity to hot and cold
• Aching pain on brushing
• Tooth sensitivity

Treatment

Correct bad habits such as horizontal brushing; treat shallow defects with desensitization, and restore marked defects with resin fillings.

Dietary advice

Avoid overly cold, hot, and acidic foods, adopt a mild soft diet, and reduce irritation.
