Perforated Peptic Ulcer

Overview

A serious complication in which a gastric or duodenal ulcer erodes deeply through the serosa, rupturing the digestive tract. Leakage of gastrointestinal contents into the peritoneal cavity causes sudden severe pain and acute diffuse peritonitis; it is a surgical emergency.

Symptoms

• Sudden severe upper abdominal pain
• Abdominal muscle rigidity
• Board-like abdomen
• Nausea and vomiting
• Fever

Treatment

A surgical emergency usually requiring surgical repair and anti-infective therapy, though some mild cases may be managed conservatively. Specialist evaluation is required.

Dietary advice

Fast after surgery, then progress from a liquid diet to an easily digestible low-residue diet during recovery, avoiding spicy foods.
